On the commercial side the dominant Blue plan here trades as Blue Cross and Blue Shield of Oklahoma, a Division of Health Care Service Corporation, a Mutual Legal Reserve Company, an Independent Licensee of the Blue Cross and Blue Shield Association, which matters mainly because its portal and its appeal route are the ones your seat will spend the most time in. Payment will not be made on claims when more than 6 months have elapsed between the date the service was provided and the date of receipt of the claim by the Fiscal Agent. OAC 317:30-3-11 — Timely filing limitation

A claim past that date is not late, it is unpayable, and no appeal recovers it.
