Mississippi Medicaid applies hard countable service caps that most states do not: physician office visits are limited to '16 per fiscal year' on a state fiscal year running 'July 1 through June 30' (not the calendar year), and prescriptions are capped at '6 per month with no more than 2 of the 6 being brand name drugs.' Billers must track visit counts against a July–June year or claims deny once the cap is hit mid-treatment. Source

On the commercial side the dominant Blue plan here trades as Blue Cross & Blue Shield of Mississippi, A Mutual Insurance Company, which matters mainly because its portal and its appeal route are the ones your seat will spend the most time in. Source

How long you have to file a Mississippi Medicaid claim

365 calendar days from the date of service. Not stated in Rule 1.6; a claim recouped by a CCO after the 365 days may be refiled within 90 days of the recoupment.

A claim past that date is not late, it is unpayable, and no appeal recovers it.
