Maine’s Medicaid program is MaineCare. Practices with a meaningful MaineCare mix usually need it worked as its own queue rather than mixed into commercial follow-up, because the submission path, the timely filing window and the appeal route are all different.

MaineCare, in practice
MaineCare has no comprehensive risk-based Medicaid MCOs — KFF's 2024 managed care table reports Maine as "N/A" with the footnote "N/A: Not applicable. State had no contracts with comprehensive MCOs," so providers bill the state directly rather than a plan network. Source: KFF
When a commercial claim is denied because it did not conform to Maine's claim-form/undisputed-claim requirements, the provider is statutorily BARRED from balance-billing the patient and must fix and resubmit: "If a claim does not conform to the requirements specified in subsections 2-A and 2-C and payment is denied to a health care provider or health care facility by a carrier, the health care provider or health care facility may not request payment from the insured or beneficiary and shall attempt to rectify the deficiencies with the claim and resubmit the claim to the carrier." For authorisations it decides who you ask. A carve-out means the rules, the portal and the turnaround belong to a different administrator from the one handling the rest of the member’s care, and an authorisation obtained from the wrong one is not an authorisation.

Maine runs on Eastern time, 10 hours behind our floor in Rawalpindi. To be at a desk for your nine to five, the shift here runs 7pm to 3am, and 6pm to 2am once the clocks go forward. Pakistan does not observe daylight saving, so the shift moves on this side rather than yours.

When does a Maine payer have to pay a clean claim?
30 days. Maine's statute makes no distinction between electronic and paper claims; a claim neither disputed nor paid within 30 days is overdue and bears interest at 1.5% per month. The authorization gets the service approved; this is the deadline for paying the claim that follows it, once that claim is clean.
How fast must MaineCare decide a prior authorization?
Under the federal CMS-0057-F rule, from 1 January 2026: 72 hours for an expedited request and seven calendar days for a standard one, for Medicaid fee-for-service and Medicaid managed care plans alike, counted in calendar time whether the request went by portal, fax or phone.
