What is specific to Washington

Apple Health is administered through managed care organizations, each with its own authorization pathway.

Washington’s Medicaid programme is Apple Health. Practices with a meaningful Apple Health mix usually need it worked as its own queue rather than mixed into commercial follow-up, because the submission path, the timely filing window and the appeal route are all different.

In Washington the queue that costs the most is Apple Health: 365 calendar days from the date of service, with a tcn assigned, and a claim past that date is unpayable rather than late.

Most of the Washington work we see comes out of Seattle, Spokane and Tacoma, though nothing about a seat depends on where your office is — the queue is worked through your system, not from your building.

Washington borders 2 states, and practices near those lines almost always carry at least one out-of-state Medicaid enrollment. That is a separate portal, a separate filing window and a separate appeal route from Apple Health, and it is the queue that most often goes unworked because it belongs to nobody.

What Washington law gives you

30 days — but expressed as a monthly-volume standard, not a per-claim deadline: 95% of the monthly volume of clean claims within 30 days, and 95% of the monthly volume of all claims paid or denied within 60 days (WAC 284-170-431(2)(a)).

Apple Health, in practice

Washington Apple Health (Medicaid) is delivered mainly through Integrated Managed Care; HCA's 2026 Apple Health Managed Care Plan Comparison Guide lists five plans — Coordinated Care of Washington, Community Health Plan of Washington, Molina Healthcare of Washington, UnitedHealthcare Community Plan, and Wellpoint. Washington is a monopolistic workers' compensation state: there is no private workers' comp carrier to bill. L&I states, 'Washington State does not allow private workers' compensation coverage. You must purchase your coverage from L&I or be a certified self-insured employer.' Every work-injury claim therefore goes to the Department of Labor & Industries State Fund or to a certified self-insured employer, on L&I's own rules and fee schedule — not to a commercial WC payer as in most states. On the commercial side the dominant Blue plan here trades as Two Blue licensees: Premera Blue Cross (Blue Cross license) and Regence BlueShield (Blue Shield license). Clark and Cowlitz counties are served by Regence BlueCross BlueShield of Oregon., which matters mainly because its portal and its appeal route are the ones your seat will spend the most time in. How long you have to file an Apple Health claim

365 calendar days from the date of service, with a TCN assigned. Resubmit, modify or adjust an initial claim within 24 months of the date of service (15 months for prescription drugs).

A claim past that date is not late, it is unpayable, and no appeal recovers it. It is the one queue where a week of nobody working it is money that does not come back — which is the argument for working Apple Health as its own queue rather than mixing it into commercial follow-up. Working Washington hours

Washington runs on Pacific time, 13 hours behind our floor in Rawalpindi. To be at a desk for your nine to five, the shift here runs 10pm to 6am, and 9pm to 5am once the clocks go forward. Pakistan does not observe daylight saving, so the shift moves on this side rather than yours.

That is the part that decides whether follow-up happens at all: a payer’s phone lines are open on Washington hours and nobody else’s.
