Minnesota delivers most Medical Assistance through prepaid managed care contracts, and its commercial and Medicare Advantage markets remain concentrated among regional nonprofit health plans rather than the large national carriers.

Minnesota’s Medicaid programme is Medical Assistance (Minnesota Medicaid). Practices with a meaningful Medical Assistance (Minnesota Medicaid) mix usually need it worked as its own queue rather than mixed into commercial follow-up, because the submission path, the timely filing window and the appeal route are all different.

That is a separate portal, a separate filing window and a separate appeal route from Medical Assistance (Minnesota Medicaid), and it is the queue that most often goes unworked because it belongs to nobody.

Medical Assistance (Minnesota Medicaid), in practice

Medical Assistance is delivered mostly through capitated managed care; the DHS Managed Care Manual states verbatim: 'The State contracts with eight MCOs to provide medical services statewide for PMAP and PMCRE enrollees,' three of which are county-based purchasing entities (South Country Health Alliance, PrimeWest Health System, Itasca Medical Care); PMAP+, MSHO, MSC+ and SNBC all run through those plans. Minnesota imposes a STATUTORY six-month timely-filing deadline that overrides nothing but binds everyone, and missing it kills the money entirely — you cannot bill the patient. Minn. Stat. 62Q.75 subd. 3(a): providers 'must submit their charges to a health plan company or third-party administrator within six months from the date of service or the date the health care provider knew or was informed of the correct name and address of the responsible health plan company or third-party administrator, whichever is later.' Subd. 3(b): a provider who misses it 'shall not be reimbursed for the charge and may not collect the charge from the recipient of the service or any other payer.' Subd. 3(g) extends the same rule to workers' compensation payers and no-fault reparation obligors. Offsetting protection in subd. 4: once a clean claim is paid, contracts must give a 12-month deadline on adjustments and recoupments. Source

On the commercial side the dominant Blue plan here trades as Blue Cross and Blue Shield of Minnesota and Blue Plus — verbatim from the plan's own site: 'Blue Cross® and Blue Shield® of Minnesota and Blue Plus® are nonprofit independent licensees of the Blue Cross and Blue Shield Association.', which matters mainly because its portal and its appeal route are the ones your seat will spend the most time in. We have not been able to source Minnesota’s own Medicaid filing limit from the regulation itself, so this page does not quote one — the federal floor is twelve months from the date of service and a state may set a shorter window. Working Minnesota hours

Minnesota runs on Central time, 11 hours behind our floor in Rawalpindi. To be at a desk for your nine to five, the shift here runs 8pm to 4am, and 7pm to 3am once the clocks go forward. Pakistan does not observe daylight saving, so the shift moves on this side rather than yours.
