Iowa’s Medicaid program is Iowa Medicaid (IA Health Link). Practices with a meaningful Iowa Medicaid (IA Health Link) mix usually need it worked as its own queue rather than mixed into commercial follow-up, because the submission path, the timely filing window and the appeal route are all different.

Medicare here is adjudicated by WPS Government Health Administrators, jurisdiction J5 — shared with Missouri and Nebraska, but not with Illinois (National Government Services), Minnesota (National Government Services), South Dakota (Noridian) or Wisconsin (National Government Services). What Iowa law gives you

30 days for clean claims — no electronic vs. paper distinction (the statute, Iowa Code 507B.4A, sets no number itself; the 30-day deadline lives in the implementing rule, Iowa Admin. Code 191—15.32(2)(a)). Interest of 10% per annum accrues from the 31st day.

Almost all Medicaid members are in the Iowa Health Link managed care program and choose one of three MCOs — Iowa Total Care, Molina Healthcare of Iowa, and Wellpoint Iowa — with dental benefits run through separate dental carriers (Delta Dental, DentaQuest) rather than the MCOs. Iowa workers' compensation has no fixed medical fee schedule: disputed charges are referred to the workers' compensation commissioner, and a provider treating a compensable injury 'agrees to be bound by such charges as allowed by the workers' compensation commissioner and shall not recover in law or equity any amount in excess of charges set by the commissioner.' How long you have to file an Iowa Medicaid (IA Health Link) claim

365 days from the date of service. Resubmit or adjust within 365 days of the last adjudication (the remittance date of denial), never beyond two years from the date of service.

A claim past that date is not late, it is unpayable, and no appeal recovers it.
