Indiana Medicaid fee-for-service has a 180-day filing limit measured from the date of service (date of discharge for inpatient), not the 12 months most states allow. Verbatim from the official IHCP Claim Submission and Processing module (Section 11, published Feb. 24, 2026): "Providers must submit claims to the Indiana Health Coverage Programs (IHCP) within 180 calendar days of the date the service was rendered. For inpatient claims, the 180-day limit is based on the member's date of discharge. See the Indiana Administrative Code 405 IAC 1-1-3 for the complete rule narrative about filing limits." On the commercial side the dominant Blue plan here trades as Anthem Blue Cross and Blue Shield (Elevance Health, Inc. is the BCBS licensee for Indiana), which matters mainly because its portal and its appeal route are the ones your seat will spend the most time in. Source
