New Hampshire Medicaid, in practice
Statewide mandatory managed care (Medicaid Care Management): "Medicaid Care Management (MCM), or managed care, delivers New Hampshire Medicaid health benefits and additional services through contracts between the Department of Health and Human Services (DHHS) and managed care organizations (MCOs)... In New Hampshire, there are three Medicaid Health Plans to choose from: AmeriHealth Caritas New Hampshire, NH Healthy Families, WellSense Health Plan." Source: dhhs.nh.gov
Several service lines are carved OUT of the MCOs and must be billed fee-for-service to NH Medicaid/DHHS rather than to the member's health plan: "Some of your services will stay under New Hampshire Medicaid through the DHHS. These include children's dental care, nursing home, and home and community-based care (HCBC) services." A biller who routes a pediatric dental or HCBS claim to AmeriHealth/NH Healthy Families/WellSense will have it rejected. Source: dhhs.nh.gov
For enrolment that is not a detail, it is a second application. A carved-out benefit is administered by a different organisation, which credentials separately, on its own timetable, and a provider who is live everywhere else can still be out of network for that one service without anybody noticing until the first claim is denied.
Working New Hampshire hours

When does a New Hampshire payer have to pay a clean claim?
15 calendar days for a clean electronic claim; 30 calendar days for a clean non-electronic (paper) claim. If the insurer denies or pends, it has 15 days (electronic) / 30 days (paper) to notify; after receiving requested additional info it must adjudicate within 45 calendar days, and if the notice is not given the claim is deemed clean. It runs from receipt of a clean claim, so claims held back while a provider’s enrollment is pending have not started it.
How often do New Hampshire providers have to revalidate?
Medicare: every five years, with sixty days to submit once National Government Services sends the notice (42 CFR 424.515). New Hampshire Medicaid: federal rule requires the state to revalidate every provider at least every five years (42 CFR 455.414), and the state agency sets the exact schedule. A Medicare revalidation left unanswered deactivates billing privileges, and claims stop with it.
