Medicare revalidation

Prepared for you, submitted with you

A fixed $249 per provider, prepaid. We collect your provider data, prepare and check every field of the CMS-855, and your own authorised official reviews and submits it — on a 20-minute screen-share with us, or by mailing the signed paper form. We never log in with your credentials and never sign on your behalf.

Ordering opens soon. Our first graded filer has to pass a timed test on real CMS paperwork before we take a prepayment for a filing (decision D13). Leave your details below and we will write to you the moment it opens — nothing is charged until then.

Ordering is not open yet — our first graded filer has to pass a timed test on real CMS paperwork first. Leave your details and we will write the moment it opens.

This form sends exactly what you type above, to the revalidation desk. No patient information, no Social Security Number, no date of birth — this is not a safe place for any of them, and the order is refused if it looks like it contains one.

What "prepared for you, submitted with you" means

  1. You give us the provider(s)’ own enrollment and practice data — never a patient’s, never a Social Security Number, never a date of birth.
  2. We prepare a complete CMS-855I or CMS-855B, field by field, against CMS’s own published forms and instructions.
  3. A second, independent person checks every field before it is considered ready.
  4. Your own authorised official reviews it and submits it themselves — CMS’s own rule is that only the practitioner, or an Authorized/Delegated Official, may sign.

Terms

What this is not: not a promise that CMS approves your revalidation, and not "filed for you" — we prepare and support submission; the practice submits it. Not a Business Associate relationship, and no patient data may pass through it.

Payment: $249 per provider, prepaid, before work begins. We will send the invoice to the email you give; nothing is prepared until it is paid.

What we never collect: your Social Security Number or date of birth, even though CMS’s own forms ask for both — those are entered directly into PECOS or the paper form, never into our systems.

Data retention: the personal data collected for the job is deleted within 30 days of the job reaching a final status.

Refund: if CMS rejects the filing because of an error we made preparing it, the fee for that provider is refunded in full.

Authority: ordering confirms you are authorised to sign or approve this revalidation on the practice’s behalf, the same authority CMS itself requires.

Questions

Do you file this with CMS for us?

No. We prepare and check every field of the CMS-855, and a second person verifies it before it is ready. The practice’s own authorised official reviews it and submits it themselves — on a 20-minute screen-share with us present, or by mailing the signed paper form. CMS itself says only the practitioner, or an Authorized/Delegated Official, may sign; we are never that signer.

What happens to our information afterwards?

The provider and practice data we collect for the job is deleted within 30 days of the job reaching a final status. See the terms below.

Do you ever ask for a Social Security Number or date of birth?

No, never. CMS’s own forms ask the provider for both in the same section — those go directly into PECOS or onto the paper form by the practice itself, never into anything we hold.

What if CMS rejects it?

If the rejection is because of an error we made preparing the form, the fee for that provider is refunded in full. A rejection caused by information the practice gave that we could not have known was wrong is not covered.

Is there a free way to check our deadline first?

Yes — the lookup at /revalidation-lookup/ answers that with no form and no charge, using the same CMS file this order page checks each NPI against.

Twenty minutes on a call is enough to tell whether this fits. If it does not, I will say so.
