What it costs

What AR follow-up and denial management cost to outsource

AR follow-up is sold four ways: a contingency share of what is recovered from old AR (OmniMD’s page puts such projects at 15 to 25 percent of what is collected from the aged balance), a percentage of all collections for full-service billing (4% to 10% of collections, per Neolytix), an hourly or monthly full-time person, or a flat seat. Ours is $1,700 a month for one full-time AR caller working denials in your own system.

The ways this work is priced

Two of these models charge on money collected, two on time. Which is cheaper depends on how much is recoverable, and that is the number to establish first.

Contingency or a seat: the arithmetic

A contingency fee is a share of recovered money, so the comparison runs the other way. At a 25 percent fee, the top of the range OmniMD describes, a $1,700 seat costs the same as the fee on $6,800 of recovered payments a month; at 15 percent, on $11,333. If more than that comes back each month, the seat is cheaper.

Before either, find out how much of the old AR is still collectable. A free AR audit answers that from two exports with no patient identifiers.

The deadlines that decide what is still recoverable

For Medicare, a claim must be filed within 1 calendar year after the date of service (42 CFR 424.44, with listed exceptions). A redetermination must be requested within 120 calendar days from receipt of the notice of the initial determination, and receipt is presumed five calendar days after the notice date (42 CFR 405.942).

Commercial payers set their own limits in their contracts and manuals. Our timely filing page lists them by payer with a deadline calculator.

What our seat includes, and what it does not

One full-time AR caller, $1,700 a month, working your worklist in your own system on US business hours.

  • Worklist worked from the difficult end, not the easy end
  • Every call logged with the payer reference and the outcome
  • Denials categorised so the same reason stops recurring
  • Appeals written and submitted, not just re-bills
  • A weekly written summary of what moved and what did not
  • A recovery percentage promise: what is collectable depends on the claims, not on us
  • Writing off a balance without your sign-off
  • Patient balance calls, which is a separate patient billing seat

When outsourcing AR follow-up is the wrong call

  • A backlog small enough for your own biller to clear in a week
  • Claims already past every filing and appeal limit: nobody can recover those
  • A payer or Medicaid contract that restricts offshore access: our seats work from Rawalpindi, Pakistan, so check first

Questions

What do AR recovery companies charge?

For aged-AR cleanup projects, OmniMD’s page describes 15 to 25 percent of what is collected from the aged balance. Full-service billing companies that include AR follow-up publish 4% to 10% of collections (Neolytix) or 4% to 8% of collections (AdvancedMD), charged on all collections.

Is a flat seat cheaper than contingency?

It depends on how much comes back. Divide the seat’s monthly price ($1,700 for ours) by the contingency rate: if more than that is recovered each month, the seat costs less.

How far back can Medicare claims still be worked?

A Medicare claim has to be filed within 1 calendar year after the date of service under 42 CFR 424.44, subject to its exceptions, and an appeal of a denial within 120 calendar days from receipt of the notice under 42 CFR 405.942.

Does the AR seat work denials or only unpaid claims?

Both. The seat calls payers on unpaid claims and reads, appeals and tracks denials, so the same denial reason stops coming back.
