In a value-based contract the practice is scored on what is documented and what is closed, not on what the clinician knows. Conditions managed all year go unrecorded because nobody built the list before the visit, and measures fail because the result sat in an outside portal or a fax queue and never reached the chart. Left alone, the gap list is rebuilt from scratch every month and the same patients fall through it. The seat does the chasing and the preparation so your coder or clinician only has to review and decide.
What the seat actually does
- Builds the pre-visit gap list from your reports, payer and registry files and the chart: conditions coded in earlier years with nothing yet this year, and measures still open.
- Retrieves the supporting records — outside notes, laboratory and imaging results, specialist letters — from portals, fax queues and the offices that hold them.
- Assembles each open item with its evidence attached, the date and where it sits in the chart, so your coder or clinician reviews and decides instead of hunting.
- Calls the patient side: booking visits and outstanding screenings, and recording what a patient says when they decline so the clinician can document the exception.
- Files the retrieved result to the chart for clinician review and records closure in the discrete field the measure reads, not as loose text the report will miss.
- Tracks every gap to a recorded outcome with a name and a date against it, so the list shrinks instead of being rebuilt from scratch each month.

How a seat starts
You issue one named login. We sign the Business Associate Agreement before it is used. We agree the numbers you will judge the seat on, which you pick and you measure. Then we train for about a week on your workflow and the seat starts. Every Friday you get a written report of what was done, what the numbers were, and what went wrong.

Is there a minimum contract?
One seat for one month, cancellable with thirty days notice. There is no annual lock-in.
Do we have to change systems?
No. The work happens inside whatever you already use, through a named login you issue. We do not sell software and we do not route your data through anything of ours.
How quickly can a seat start?
About a week from the agreement, which covers training on your workflow, your payer mix and your system. You are not charged extra for that week.
Does the seat assign the diagnosis codes?
No, and it should not. The seat is a trained non-clinical coordinator working offshore on your hours, not a credentialed coder or a clinician. It finds the gap, pulls the record that speaks to it, and puts that evidence in front of your reviewer with the date and the location in the chart. Your coder or the treating physician decides what the record supports and signs. Risk adjustment carries audit exposure, and the decision has to sit with the person who can defend it. If you want a supplier who will assign final codes without your review, we are not that supplier.
Our results come from outside — hospital portals, reference laboratories, other practices. Can the seat actually get them?
Yes, wherever access can be granted, and access is the real constraint. The seat works your portals under its own named login, calls the outside office and stays on the line until someone sends the record, and works the fax queue. Some hospital systems and reference labs will not credential a third party, or block logins from outside the US; where that happens the seat prepares the request and someone in your office sends it. It cannot do anything physical either — it cannot walk a release to a records desk or collect a disc.
How is this different from hiring a coding vendor?
A coding vendor takes a finished encounter and codes it. This seat works before and around the encounter — building the gap list the clinician sees at the start of the day, getting the supporting record into the chart while it still matters, and tracking what closed afterward. It is one named person on your hours inside your system, not a batch handed back later. If what you need is a retrospective chart review signed off by credentialed coders, a coding vendor is the better fit and we will say so.
Is our practice too small for this to make sense?
Probably, if only a small share of your patients sit in a value-based contract, or your measure set is short enough that your front desk already closes it between calls. A seat is $2,300 a month for one full-time person, and that is only sensible when there is a full day of gap lists, record retrieval and follow-up in it. If the work is half a day, ask for the seat to carry another queue as well rather than paying for idle hours.
How do we see that the work is being done, and what does the seat not touch?
Every gap on the list carries a state and a name: what was attempted, when, who was spoken to, and what came of it. You work from the same tracker the seat does, in your system wherever your system allows it. The seat works US hours, so the queue moves during your day and questions get answered while your office is open. What it does not do is sign, attest or submit anything in your name, and it is not your compliance officer — those stay with your people. Paid up front, one month minimum, thirty days' notice if it is not earning its place.
