Chronic care management is billed month by month, and only if someone enrols the patient, records the consent, makes contact each month, and logs the time against a care plan. Remote monitoring is the same: readings arrive, and someone has to notice the ones that matter, call the patient who has stopped sending them, and record the review. It often goes unbilled, because the staff who could make those calls are already busy with the people in front of them. The seat is the person who makes the calls.

What the seat actually does

  • Enrollment calls in the wording your practice approves, consent captured and recorded where you keep it, refusals logged so the same patient is not called twice.
  • Care plan typed into your template from the chart and your clinician's instructions, then handed back for that clinician to amend and sign. The clinical content stays theirs.
  • Monthly outreach run off a calendar rather than memory: scheduled calls, repeat attempts at different times of day, each attempt logged with what was discussed.
  • Every minute logged in your system against the patient on the day it is worked, so the month's total is a contemporaneous record rather than a month-end reconstruction.
  • Device readings checked each working day against the thresholds you set, patients followed up when readings stop, out-of-range values passed to your clinician uninterpreted.
  • Month-end list: who met the threshold your practice bills on, who fell short and why, whose consent has lapsed, whose device has stopped transmitting.

How a seat starts

You issue one named login. We sign the Business Associate Agreement before it is used. We agree the numbers you will judge the seat on, which you pick and you measure. Then we train for about a week on your workflow and the seat starts. Every Friday you get a written report of what was done, what the numbers were, and what went wrong.

Is there a minimum contract?

One seat for one month, cancellable with thirty days notice. There is no annual lock-in.

Do we have to change systems?

No. The work happens inside whatever you already use, through a named login you issue. We do not sell software and we do not route your data through anything of ours.

How quickly can a seat start?

About a week from the agreement, which covers training on your workflow, your payer mix and your system. You are not charged extra for that week.

Can the seat do the clinical part — the assessment, the medication review, the care plan itself?

No, and it will not pretend to. The seat is not a licensed clinician, holds no US license, and makes no clinical judgment. It gathers what your clinician needs — what the patient said this month, what has changed, which readings fall outside the range you set — and puts it in front of that clinician to decide on and sign. Where a licensed or credentialed person has to own the decision and the record, that person still owns it. The seat prepares the work, follows up with the patient, and keeps the documentation straight.

How is the time logged, and does that time count toward what we bill?

Time is entered in your system, against the patient, on the day the work happens, with a note of what was done. The limit you need to know: this seat is not licensed clinical staff, and these programmes generally require the billable time to be furnished by the practitioner or by clinical staff working under that practitioner's supervision. Whether an offshore contractor's time counts toward the threshold you bill on is your practice's determination with its own compliance counsel, not ours, and some practices decide it does not. We log the work either way, so you have the record and see shortfalls during the month rather than after the claim.

Is this worth a full-time seat for a small practice?

Often not, on this work alone. If your eligible list is small enough that one person could call everybody in a couple of days, a seat dedicated to chronic care will sit idle most of the month and you will be paying for the idle time. For a small practice the honest answer is either to run the seat across other back-office work as well — eligibility checks, prior authorization follow-up, recall lists, records requests — or to leave this until the list is bigger. We would rather say that now than three months in.

Who do patients think they are speaking to, and when do the calls happen?

The seat works US hours in your time zone and calls out through your phone system, so the number the patient sees is your practice's. It introduces itself with the wording you approve, as part of your care team, and follows your script. If a patient asks directly where the caller is based, the seat answers honestly: trained staff in Pakistan working for your practice. Nothing is scripted to hide that, because a patient who finds out later stops trusting the call.

What does $1,700 cover, and what do we have to provide?

It covers one full-time person on your hours, working inside your systems under your direction, paid up front, one month minimum, thirty days' notice. You provide access with the permissions the work needs, your consent wording and escalation rules, and a named clinician who will review and sign care plans and act on the readings that need a decision. That last part is not optional. Without someone on your side who answers questions and signs, the work stops at the desk and nothing gets billed.
